Insurance and Payment methods

Note: Dr. Goodman does not participate with Medicare and is NOT in-network with UHC.  United Healthcare (not Oxford) does not credential NDs, so those with UHC may be eligible for reimbursement out-of-network

Cancellation/Rescheduling policy: Please be aware that there is a 24-hour cancellation policy.  The time reserved for your appointment is valuable, and notice given allows the office to allocate your time slot to someone else in need of care.

No-shows or appointments canceled with less than 24 hours notice will result in a $90 fee. Cancelations can be made by phone only.  If an emergency occurs and you cannot make an appointment this policy will not be in effect the first occurrence. A $90 deposit will be placed on your credit card to secure your first appointment.

Credit Card on File Policy

The office has a policy of keeping your credit or debit card on file as a convenient way for you to pay for the portion of your services not covered by insurance, but for which you are liable.  Our time here in the office is spent providing superlative health care and supporting patients interested in optimal health.  Consistent with these priorities, we will not accept new patients unwilling to provide credit/debit card authorization.  The office will ask for your card information when you make your first appointment, and a form authorizing the office to keep your card on file is included in your new patient documents.

Your information is kept confidential and secure in a Payment Card Industry Data Security Standard (PCI DSS) compliant database.

Individuals with insurance plans that we don’t accept (including Medicare) must pay for services at the time they are rendered. If you have a private insurance plan (non-HMO), then your insurance company will be responsible for providing some type of out-of-network coverage. Please contact your insurance directly for their out-of-network policy.

Regarding payment: Payment is required at the time of services rendered. We accept cash payments, personal checks and Mastercard/Visa/AmEx/Discover.

Fees:
New Patient appointments: $180 (approx. 1 hour)
Return patient appointments: $125 (approx. 30 minutes)
